慢性颈肩背部疼痛如何与颈椎病相鉴别?

有些病人因为反复的颈肩背部产而怀疑自己是否得了颈椎病(什么是颈椎病？)。其实引起慢性颈肩背部疼痛的原因很多，例如颈部扭伤、外伤、肩关节周围炎、落枕、慢性肌纤维织炎、筋膜炎、韧带劳损等软组织疾患，均可引起颈肩背部慢性肌紧张性疼痛。此外，畸形、感染、肿瘤、风湿性炎症、结核等也可以引起慢性颈肩背部疼痛。那么，如何才能与颈椎病相鉴别呢？

颈部扭伤和落枕往往由于颈部活动不当或睡眠中体位不良以致局部肌肉被扭伤。压痛点不像颈椎病一样位于棘突部，而是位于肌肉损伤部，以肩胛内上方多见，且伴有肌肉痉挛，可触及压痛明显的条索状痉挛肌束，颈部牵引不像颈椎病一样症状消失或缓解，反而疼痛加剧；对封闭疗法则有明显效果。

肩关节周围炎疼痛往往在肩关节处，颈部症状仅为受牵拉后产生。其特征性表现为肩关节活动明显受限，X线片表现无明显颈椎病理改变。但有时与颈椎病伴发，在诊断上易造成混淆。

肌纤维织炎则有风寒、潮湿、劳累、扭伤等明显诱因。局部肌肉僵硬，以酸痛为主，范围较大，多无固下压痛，按揉有明显舒适感。

总之，是否有颈椎病，可根据患者发病情况、诱发因素、病史、体征、X线片及其它辅助检查加以区别，尤其是X线片，在鉴别诊断上具有较高的价值。

  20. On re-reading Josephus and Philo, it appears that Josephus does say they “bathe their bodies in cold water,” and that this is a “purification” (Wars 2.8.5). This sounds like it could be daily baptism, depending on whether this is done for ritual purposes or just to become clean. Philo, Every Good Man is Free 12, doesn’t discuss bathing or baptism.
